Significance of PFOA blood test results for people living nearby DuPont/Chemours

Samenvatting

De langdurige blootstelling aan perfluoroctaanzuur (PFOA) in het verleden heeft bij omwonenden van de DuPont/Chemours fabriek mogelijk gezondheidseffecten veroorzaakt. Dit mede omdat het aannemelijk is dat de concentraties PFOA in bloed in het verleden aanzienlijk hoger zijn geweest dan nu worden gemeten. De uitkomst van het bloedonderzoek bevestigt de eerdere modelberekeningen. Omwonenden die lang dichtbij de fabriek wonen hebben hogere concentraties PFOA in bloed dan bewoners die verder weg wonen of minder lang in de omgeving wonen. Deze resultaten zijn in lijn met een eerdere risicoschatting van het RIVM uit 2016. Hierbij werd berekend dat het waarschijnlijk is dat de gezondheidskundige grenswaarde voor PFOA langdurig is overschreden. Uit een analyse van diverse epidemiologische studies blijkt dat er relaties zijn tussen ongewenste veranderingen in het lichaam en de PFOA concentratie in het bloed. Veranderingen worden al waargenomen vanaf relatief lage PFOA bloedconcentraties zoals die voorkomen bij de algemene bevolking. Een relatie is het meest waarschijnlijk voor veranderingen in cholesterol, leverenzymen en geboortegewicht. In de wetenschap is nog veel onzekerheid bij welke bloedconcentraties deze ongewenste veranderingen in het lichaam optreden. Op basis van de beschikbare epidemiologische informatie is het niet mogelijk een oorzakelijk verband aan te tonen tussen de ongewenste veranderingen en PFOA blootstelling. Het RIVM verwacht dat een (individueel) gezondheidsonderzoek onder omwonenden weinig tot geen gezondheidswinst oplevert voor de omwonenden. Herkenning en behandeling van de ongewenste veranderingen in het lichaam die mogelijk als gevolg van PFOA blootstelling optreden, worden in sommige gevallen al ondervangen door standaard controles. De mogelijke ernstige gezondheidseffecten (zoals nierkanker, testiskanker en colitis ulcerosa) komen heel weinig voor. Hierdoor is de kans klein dat bij een screening op deze ziektes extra nieuwe gevallen zullen worden opgespoord. Een aantal van de mogelijke ongewenste veranderingen veroorzaken gezondheidseffecten die men tijdig met de huisarts zal bespreken. Mensen die zich zorgen maken over hun gezondheid wordt aangeraden contact op te nemen met hun huisarts.

Abstract

Long-term exposure to perfluorooctanoic acid (PFOA) in the past may have affected the health of people living nearby the DuPont/Chemours factory. This is partly because it is likely that the concentrations of PFOA in the blood were substantially higher in the past than have now been measured. The outcomes of the blood tests confirm previous model calculations. Residents who have lived in the vicinity of the factory for a long period of time have higher concentrations of PFOA in their blood than residents who live further away or who have lived in the vicinity for shorter periods of time.

These results are in line with an earlier risk assessment carried out by RIVM in 2016. The calculations revealed that it is probable that the health-based threshold value for PFOA was exceeded for a long period of time.

An analysis of various epidemiological studies shows that there are links between undesirable changes in the body and the PFOA concentration in the blood. Changes are noticeable even at relatively low PFOA blood concentrations, as occur in the general population. A link is most probable for changes in cholesterol, liver enzymes and birth weight.
Scientists are still unsure about the exact blood concentrations at which these undesirable changes occur in the body. On the basis of the available epidemiological information it is not possible to demonstrate a causal relationship between the undesirable changes and PFOA exposure.

RIVM expects that an (individual) health study among local residents will produce little to no health benefits for the people in question. In some cases the undesirable changes in the body which may occur as a consequence of exposure to PFOA can already be detected by standard controls and can therefore be treated. The possible serious health effects (such as kidney cancer, testicular cancer and ulcerative colitis) occur only on a very limited scale. As a result, the chance is small that a screening for these conditions will lead to the detection of any new cases. A number of the possible undesirable changes cause health effects that will be discussed in good time with GPs. People who are concerned about their health are advised to contact their GP.
