Samenvatting

Er waren in 2012 geen grote uitbraken van de meldingsplichtige luchtweginfecties legionellose, psittacose, Q-koorts en tuberculose. Wel duurde de influenza (griep)epidemie van het seizoen 2012/2013 uitzonderlijk lang: met 18 weken was het de langstdurende epidemie in de afgelopen 25 jaar. Mogelijk heeft dit geleid tot meer longontsteking en meer sterfgevallen. Griep en longontsteking leiden jaarlijks tot veel ziekteverzuim en huisartsenbezoeken. Ook zijn ze een belangrijke oorzaak van ziekenhuisopname en sterfte. De meldingsplichtige luchtweginfecties komen veel minder voor dan griep of longontsteking in het algemeen. Ze zijn meldingsplichtig, omdat tijdige maatregelen, zoals de besmettingsbron opsporen, belangrijk kunnen zijn om uitbraken of verdere verspreiding van de ziekte te voorkomen. Het RIVM voert met partners continue surveillance uit om ontwikkelingen in luchtweginfecties tijdig te signaleren. Meldingsplichtige longontstekingen De meldingsplichtige longontstekingen vormen een klein deel van het totaal aantal longontstekingen. In 2012 was het aantal longontstekingen veroorzaakt door de legionellabacterie (veteranenziekte) met 304 meldingen vergelijkbaar met vorig jaar. Bijna de helft van de patiënten had tijdens de incubatietijd een reis gemaakt; vooral bij een reis naar Italië was het risico op een legionella-infectie hoger. Er waren 46 meldingen van psittacose, een vorm van longontsteking waarbij vooral vogels de bron van infectie zijn. Dit is het laagste aantal sinds 2004. Er was echter wel een uitbraak van psittacose bij medewerkers van een vogelopvang. Met slechts 66 meldingen was er weinig acute Q-koorts, wat bevestigt dat de grote Q-koortsepidemie van 2007-2010 voorbij is. In Friesland werden echter meer Q-koortspatiënten gezien dan voorgaande jaren, zonder dat hier een bron van infectie gevonden werd. Tuberculose Het aantal tuberculosepatiënten in Nederland is sinds 2002 met een derde gedaald. Na een tijdelijke toename in 2009 tot 1158 meldingen, zette de daling in de jaren erna door tot 958 meldingen in 2012. Evenals voorgaande jaren is in 2012 bijna driekwart van het aantal tbc patiënten geboren in het buitenland. Vooral onder Somaliërs komt relatief vaak tuberculose voor. Griep In het griepseizoen van 2012-2013 circuleerden er vier verschillende griepvirussen: twee verschillende influenzavirussen type A, en twee soorten influenzavirus type B. De effectiviteit van het influenzavaccin was voor één virustype zeer laag en voor de andere drie soorten virussen redelijk tot goed. Tijdens de influenza-epidemie werd langdurig een verhoogd aantal sterfgevallen waargenomen, dat mogelijk deels aan de griep kan worden toegeschreven.

Abstract

In 2012 there were no large outbreaks of the notifiable respiratory infections legionellosis, psittacosis, Q-fever and tuberculosis in the Netherlands. However, the long-lasting influenza epidemic was exceptional: with 18 weeks duration it was the longest epidemic in the last 25 years. This may have led to an increase in pneumonia and increased mortality.

Influenza and pneumonia are important reasons for general practitioner consultation, and can result in hospital admission and death. The notifiable infectious diseases legionellosis, psittacosis and Q-fever mostly present as pneumonia but represent only a small proportion of the total number of pneumonia patients. They are notifiable because timely measures, like source finding, can be important to prevent outbreaks or ongoing transmission of the disease. For timely detection of developments in respiratory infections, the RIVM and partners conduct ongoing surveillance of influenza, pneumonia and notifiable respiratory infections.

Notifiable pneumonia
In 2012 there were 304 notifications of legionnaires' disease, which is comparable with 2011. Nearly half of the patients had travelled during the incubation period; especially travel to Italy was associated with an increased risk of a legionella infection. There were only 46 notifications of psittacosis, a type of pneumonia that is mainly transmitted from birds. This is the lowest number since 2004. Nevertheless, there was an outbreak of psittacosis among the staff of a bird's shelter. Acute Q-fever incidence was low with only 66 notifications, which confirms that the large outbreak of Q-fever that was seen from 2007-2010 is really over. In the province of Friesland however, more Q-fever patients were reported than in previous years. For these patients no common source of infection was found.

Tuberculosis
Since 2002 the number of TB cases in the Netherlands has decreased with 32 per cent. After a temporary increase in 2009 to 1158 notifications, the decline further continued in the following years. In 2012 958 TB cases were reported, almost half of which were extrapulmonary TB. Like previous years, nearly threequarters of TB cases are of foreign origin. The TB-rate is especially high among immigrants from Somalia.

Influenza
During the influenza-season of 2012/2013 four kinds of influenza virus were circulating: two different subtypes of influenza virus type A, and two lineages of influenza virus type B. The effectiveness of the influenza-vaccination was very low for one influenza virus A subtype and moderate to good for the three other circulating kinds of influenza virus. During the influenza epidemic a long lasting increased mortality was observed, which may partly have been related to influenza.
