Samenvatting

Gemeentelijke maatregelen op het gebied van groen in de wijk, het binnenmilieu van woningen, veiligheid, geluid van verkeer, luchtverontreiniging, sociale cohesie en sociale steun kunnen de gezondheidsachterstanden van mensen uit lagere sociaaleconomische klassen verminderen. De literatuur levert voldoende bewijs dat deze omgevingskenmerken geschikte aanknopingspunten zijn voor maatregelen om gezondheidsachterstanden te verminderen. Onderzoek toont namelijk dat genoemde omgevingskenmerken tussen hogere en lagere sociaaleconomische klassen verschillen en dat ze samenhangen met gezondheid. Dit blijkt uit een literatuurverkenning van het RIVM naar de mogelijkheden om sociaaleconomische gezondheidsachterstanden terug te dringen door lokale maatregelen in de fysieke en sociale omgeving. De gezondheid van mensen met een lage sociaaleconomische status (SES) is over het algemeen slechter dan de gezondheid van mensen met een hogere SES. Deels wordt dit verklaard door de ongezondere fysieke en sociale omgeving waarin mensen met een lage SES leven. Over effecten van concrete omgevingsmaatregelen om gezondheidsachterstanden te verminderen, zijn geen studies gevonden met hoge bewijskracht, zoals reviews of RCT's (gerandomiseerde gecontroleerde studies). Er zijn studies met hoge bewijskracht gevonden die aantonen dat betere warmte- en energiesystemen in huis, verkeersvertragende maatregelen, betere straatverlichting en opvoedingsondersteuning de gezondheid bevorderen. Deze maatregelen dragen dus positief bij aan de gezondheid van de algemene bevolking, maar de invloed op vermindering van gezondheidsachterstanden is niet onderzocht. De volgende maatregelen zijn wel specifiek bij achterstandsgroepen onderzocht maar de studies hebben geen hoge bewijskracht: verkeersmaatregelen (verkeersvrije gebieden, wandelroutes en een veilige schoolroute voor kinderen in de vorm van een 'Walking School Bus') en het inrichten van ontmoetingsplekken. Hieruit komt naar voren dat deze maatregelen lichaamsbeweging respectievelijk sociale cohesie kunnen bevorderen.

Abstract

Measures taken at local authority level regarding green spaces in the neighbourhood, indoor environment of houses, neighbourhood safety, traffic noise, air pollution, social cohesion and social support can minimise health inequalities for people with a low socio-economic status (SES). The literature sufficiently shows that these environmental factors represent suitable target areas for measures to reduce health inequalities because research has shown that the above environmental factors differ in people from higher and lower socio-economic groups and that they are also related to health.
This can be concluded from a literature survey conducted by the National Institute for Public Health and the Environment (RIVM) on the available options for reducing socio-economic health inequalities by introducing measures to improve the physical and social environment at local authority level. In general, the health of people with a lower SES is worse than those of higher SES. This can partly be explained by the physical and social environment of people of lower SES - that is unhealthier.
Regarding studies on the effects of concrete environmental measures to reduce health inequalities, no studies such as reviews or randomised controlled trials, presenting strong evidence were found. However, studies with strong evidence were found that showed that improved heating and energy in housing, traffic calming measures, better street lighting and parenting support all promote human health. These measures thus contribute positively to the health of the general population but how they help to reduce health inequalities has not been investigated.
The following measures have been specifically studied in disadvantaged groups, but these were no studies presenting strong evidence: traffic measures (such as traffic-free areas, walking routes and Walking School Bus) and the setting up of meeting points. From these studies it can be concluded that these measures promote physical activity and social cohesion respectively.
